Either design flaw or a bad shoe

Third year in a row i have purchased this show for my son who now is 16. He still wears a 9.5.
This year the shoe is chewing (blisters) up his heel about 2 inches up from bottom of foot. The opening of the shoe where you slide your foot in is larger on the right shoe and this is the foot he continuously gets blisters on. Is it poor design or a right shoe that should never have passed quality control? At $240 bucks tax in - not a happy customer. I have yet to reach out to PUMA so who knows, maybe they will be good and have me send them back.
